Gupta Saket , Hyde Sarah , Abbasi Babar , Thomas Sam , Hoashi Shu

We report a rare case of 37 years female of Greek and Irish extraction with a history of thalassemia trait, who presented with generalised weakness and severe loin and pelvic pain 9 days post vaginal hysterectomy and 3 days after hospital discharge. After readmission, she developed mild pyrexia (37.5°C), hypotension, mild hyponatraemia with plasma sodium which fell from 138 to 131 mM and early signs of acute respiratory distress syndrome. A Short Synacthen test was perfor...

Inflammation is a component of obesity-associated insulin resistance. Significant increases in inflammatory mediators such as IL6, TNFa and IL1b have been detected in the serum of diabetic patients. Herein, we investigated whether pre-diabetic patients also exhibited perturbations in circulating pro-inflammatory cytokines. To this end, we recruited 42 healthy non diabetics and subjected them to 75 g OGTT after an overnight fast. We profiled the cytokines that are present in th...

Power spectral analysis (PSA) of heart rate variability (HRV) in the detection of cardiac autonomic neuropathy (CAN) in subjects with diabetes mellitus

Khamis Amjed , Rizvi SR , Hoashi Shu , Byrne Marie , Firth RG , Kinsley Brendan

Standard Autonomic Function tests AFT may not detect subclinical CAN. Modalities exist (Frequency Domain) using PSA of HRV, which may detect subclinical CAN.We performed standard AFTs (HR response to deep breathing, Valsalva, Tilt at 1 min and 6 min) and PSA of HRV on these tests in 46 subjects (29 DM and 17 Controls) matched for age and sex. We sought to establish if those DM subjects considered normal by AFT would exhibit abnormalities in PSA of HRV. W...
